Patient self-refer
Easiest · Same dayHand the patient our card or write down unclenchdental.com. They submit the inquiry form. We follow up directly with both you and them.
Use when: the patient is engaged and self-directed.
Email referral
Recommended · 1 business dayEmail [email protected] with: patient name, DOB, phone, and a 1–2 sentence reason for referral.
We respond within 1 business day, contact the patient to schedule, and you'll receive a chart note within 5 business days of the visit.
Direct phone or fax
Urgent or complexEmail [email protected] requesting a callback. Dr. Nguyen returns provider calls personally within 24 hours.
Use when: the patient is complex, time-sensitive, or you'd like to discuss before the patient is contacted.
What your patient will experience
- Inquiry response within 2 business days from Dr. Nguyen personally.
- Intake forms by secure link — HIPAA-compliant, ~10 min to complete.
- 45-min virtual consult — full history + candidacy review. Many consults end here, by design.
- If proceeding — in-home, in-office, or hotel visit scheduled within 1–2 weeks.
- Follow-up at 2 weeks (text/telehealth) and 12 weeks (full reassessment).
What your office will receive
- Acknowledgment within 1 business day of referral
- Consult note within 5 business days of each visit
- Recommendations specific to your continued care of the patient
- Direct line to Dr. Nguyen for clinical questions
- The patient back. Always.
Our commitment to your patient relationship
The patient remains your patient. We provide a deliberately limited scope of care (therapeutic neuromodulator only). We do not market other services to referred patients. We refer them back for everything outside our scope. If a boundary ever feels crossed, tell us and we fix it within the week.
