Our HIPAA-compliant patient intake portal is being configured. Until it's live, intake forms are sent by secure link directly to accepted patients after the initial consultation.
If you're a new patient
Start with an inquiry. We review every inquiry personally within two business days. Once accepted, you'll receive your intake forms by secure link and a private booking page for your virtual consult.
Existing patients can complete intake or update forms through the patient portal. If you can't find the link in your email, or need a new one, message [email protected].
Intake forms are managed through our HIPAA-compliant practice management platform. Submitting intake does not create a clinician–patient relationship; that begins only after consultation and acceptance into the practice.
